Job Description Join our amazing Bristol Hospice Palliative Care
Team The Palliative Care Nurse Practitioner provides care through
the evaluation and assessment of the health history and diagnoses
of medical conditions. You will manage patient health problems by
directing and developing the plan of care, prescribing medications
or treatments, etc. You will build from the resources of the
community to plan and direct services to meet the need of
individuals and families within their homes and communities.

Reimbursement Percentage of reimbursement/collections will go to
YOU REQUIRED EDUCATION & EXPERIENCE: Must have a current Nurse
Practitioner licensure in the state, in good standing Must be a
graduate of an accredited School of Nursing and accredited Nurse
Practitioner Program. Accreditation from an approved certifying
body for advanced practice nursing as required by State. Must have
a Master's Degree Minimum of one year Nurse Practitioner experience
or Palliative Care experience preferred Certification in a
specialist area preferred (Palliative Nursing (CHPN), Pediatrics or
Geriatrics). JOB RESPONSIBILITIES: Provides medical care, palliates
and manages terminal illness in compliance with the state nurse
Practice Act under the supervision of a collaborating physician
Completes history and physical examinations by evaluating the
patient's medical condition and health history, ordering and
interpreting results from laboratory and diagnostic tests or
procedures, diagnosing health conditions and documents the findings
in the patient's medical record Review and develop protocols for
treatment and propose options for interventions based on the
literature in collaboration with the collaborating physician Serve
as a nursing resource for consultation and education to members of
the interdisciplinary team and other healthcare practitioners in
the community etc.
