Job Description:

The WelbeHealth Occupational Therapist is responsible for coordinating and providing restorative and rehabilitative Occupational Therapy services to participants at the Center and at home. Occupational Therapy works closely with PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety as well as enhance performance of ADLs. The Occupational Therapist collaborates with the Physical Therapist to efficiently operate WelbeHealth rehabilitation department and is a member of the WelbeHealth management team and facilitates collaborative relationships across WelbeHealth and with external stakeholders to promote the WelbeHealth values, team culture and mission..   • Conduct Occupational Therapy assessments to determine the Occupational Therapy
  • care needs, preferences and goals of the participants and actively participate in IDT
  • meetings to develop participant care plans.
  • Deliver and document Occupational Therapy interventions in accordance with physician
  • orders and/or as agreed upon in the participants' care plans including but not limited to
  • the use of devices, techniques and activities to promote muscle re-education,
  • perceptual-motor and cognitive function, balance, endurance and other factors affecting
  • the level of independence in performing ADLs.
  • Supervises the Therapy Assistants including providing training, instruction and guidance
  • needed to implement Functional Maintenance Programs (FMP).
  • Provide training and instructions to staff, participants and caregivers/families in areas of
  • Occupational Therapy in which they may work with participants.
  • Order, track and manage maintenance for equipment.
  • In coordination with the Marketing Team, support enrollment of prospective participants
  • into the program.
  • Participate in end-of-life care coordination and support.
  • Manage direct reports including training, supervising and mentoring.Qualifications and Requirements:
    • Bachelor's Degree in Occupational Therapy from Accredited Program AOTA and
    • ACOTE standards.
    • Initial Registration from National Board for Certification in Occupational Therapy.
    • Licensed and/or eligible for Licensure as required in the State of California.
    • Valid State Driver's License with a clean DMV record.
    • Copy of recent Vehicle Insurance required.
