details) Start Date: ASAP About the Position We are seeking a
compassionate and mission-driven Locums Primary Care Physician to
join our interdisciplinary team in Fresno, CA as part of our
Program of All-Inclusive Care for the Elderly (PACE). This unique,
value-based care model enables providers to build deep, long-term
relationships with a smaller panel of patients—focusing on quality,
not quantity. As a Primary Care Physician within our holistic,
team-based model, you'll experience the rewarding impact of working
with frail, underserved older adults in a setting that prioritizes
comprehensive, patient-centered care over volume-based medicine.
Compensation: $140-$160 Per/hour Travel reimbursement. Position
Highlights: Provide primary care services to a panel of geriatric
patients enrolled in the PACE program Collaborate closely with an
interdisciplinary team including nurses, therapists, social
workers, and other specialists Deliver patient-centered,
preventive, and chronic care in an outpatient clinic setting Daily
patient volume is 8 in the clinic and 4 in field per day
Participate in care planning, case management, and care
coordination for complex patients Enjoy a manageable patient load
and extended time with each participant Qualifications: MD or DO
degree required Board Certification in Internal Medicine, Family
Medicine, Emergency Medicine, or Geriatrics Active California
medical license and DEA (or ability to obtain) Must be eligible for
enrollment with Medicare and Medi-Cal Prior experience in
geriatrics or working with frail, medically complex adults
