expanding our team and are seeking compassionate, organized, and
team-oriented Licensed Vocational Nurse (LVNs) for multiple

you. Available Territories & Pay Structure We are hiring for three
separate territories. Compensation is based on experience and
territory assignment: Alameda County & West Contra Costa County Pay
Rate: $94,000 – $114,000 per year DOE Coverage Area: Oakland/San
Leandro/Berkeley/Richmond/San Pablo East Contra Costa County Pay
Rate: $94,000 – $114,000 per year DOE Coverage Area:
Antioch/Pittsburg/Brentwood TriValley Area Pay Rate: $94,000 –
$114,000 per year DOE Coverage Area:
Dublin/Livermore/Pleasanton/San Ramon/Danville Why Health Link

collaborative back-office staff Responsibilities Implement the plan
of care as initiated by the Registered Nurse Provide accurate,
timely, and complete documentation in accordance with the plan of
care Assess patient needs and deliver patient and family/caregiver
education related to the diagnosis, treatment, and plan of care
Participate in coordinating home health services, including
identifying needs for additional disciplines (HHA, OT, PT, ST, MSW)
and communicating them to the Registered Nurse and/or Clinical
Supervisor Use equipment and supplies effectively and efficiently
Complete online charting in Kinnser promptly and accurately Adhere
to all company policies and procedures Perform additional duties as

California LVN License (Required) Home Health Experience: 1 year
(Preferred) Current CA Driver's License Excellent organizational
and communication skills Ability to work independently in the field
