Physical Therapist Location: Oakland, CA Agency: Ambition 24Hours Pay: $2,915 per week Shift Information: Rotating - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Physical Therapist Skilled Nursing/Rehab Oakland, CA (7 day Availability) Role overview A Physical Therapist is needed for a full time day shift contract at a skilled nursing and rehabilitation facility in Oakland, CA, serving adult and geriatric residents needing short term rehab and long term restorative care. This 13 week replacement assignment focuses on post acute rehab in a SNF environment with an interdisciplinary, multi lingual team. Assignment details Location: Skilled nursing & rehab center, Oakland, CA 94601 Setting: Skilled Nursing Facility (SNF), short term rehab and long term care Position type: Contract, full-time, non permanent Contract length: 13 weeks (02/23/26 05/23/26) Schedule: 8:00 AM 4:30 PM, 30 minute unpaid meal break Days: Sun, Mon, Tue, Wed, Thu, Fri, Sat (actual work days per facility schedule) Guaranteed hours: 35 hours per week Gross Blended pay rate: $58.80 - $68.60 p/h Order reason: Replacement Key responsibilities Perform comprehensive PT evaluations (strength, ROM, balance, gait, transfers, functional mobility) for adult and geriatric residents. Develop and implement individualized plans of care to improve mobility, independence, and overall functional safety in the SNF setting. Provide evidence based interventions including therapeutic exercise, gait and stair training, neuromuscular re education, and balance/coordination activities. Collaborate with an interdisciplinary team (OT, SLP, nursing, social services, physicians) to coordinate rehab programs and discharge planning. Document evaluations, daily treatment notes, progress notes, and discharge summaries accurately and on time in the EMR. Educate patients and families on home exercise programs, safe mobility strategies, and use of assistive devices. Required qualifications and skills Active California PT license (or eligibility prior to start). Graduate of an accredited Physical Therapy program (DPT, MSPT, or BSPT). Prior SNF or post acute rehab experience preferred. Current BLS/CPR for healthcare providers. Strong clinical reasoning, documentation, and time management skills with the ability to manage a diverse caseload.
